1. Coordinator Mr van Vliet   (approval of your destination; come to the office hours)
2. After the approval of your destination; department leader Mrs Huisman (absence from school)
3. And finally your mentor (Targets and Key skills)

2. scan your form and submit

When all is set and done, scan all papers and send them below

  • Send in a clear scan of your initiation form, do not take a photo but use a scanner! Make sure your file does not exceed 20 MB. Use PDF or Word.

After you have submitted the form you have permission to confirm your dates and book the trip.

NOTE: Some airlines use strict rules about booking under the age of 16. Check these rules before you book the flight.
